This phrase is often used to get people to pay attention to the hazards they face while undertaking a dive, especially a rebreather dive.
I recently had a conversation with a safety professional who was looking for some advice.
Insight: We must improve our understanding of the role behaviour plays in safety performance improvement.
Insight: Leadership stimulates growth and safety improvement, culture sustains it.

A review of injury and illness data shows that Total Recordable Injuries have been trending downward for the past several years, unfortunately, fatalities are staying the same or in some industries rising.
Most executives want to achieve safety excellence; however, they are not sure how to get there.
It has certainly been a very eventful 6 months. Who would've thought as we saw in the New Year on 31st December 2019, that within three months we would be in a government instigated lockdown in a global effort to control the spread of a pandemic which was going to kill so many people? It's like a movie that you watch from the comfort of your sofa, safe in the 'knowledge' that it'll never happen for real; except it did happen for real and now, as we hopefully start to put it behind us we are left with the fallout. Families mourning their lost loved ones, workers the World over mourning their jobs, business leaders and owners mourning the loss of their turnover, profit, staff and livelihood and governments in a state of paralysis wondering what the hell just hit them and reeling from the backlash. Unfortunately, we are far from out of the woods yet.

Paradigm Human Performance recently held a webinar that outlined suggested measures business should consider adopting when staff begin returning to work. The advice was gained from health professionals who had experienced dealing with previous pandemics such as SARS, MERS and Ebola. Consequently, their experience was gained from being isolated following caring for patients rather than a mass lock-down that much of the world's population currently faces.
The two factors to consider are the physical and psychological impacts.
Recently Teresa Swinton, the Founder and Owner of Paradigm Human Performance, gave a presentation to her neighbours in the Plymouth Science Park, Plymouth, UK, and titled it Managing Operational Performance During COVID-19. There are many gems and valuable nuggets in the presentation, but one struck a chord with me when she mentioned Deference to Expertise, a principle of High Reliability Organizing. You would be well-served if you were to view the entire presentation here: https://youtu.be/ai7F-CWXuvI
Successful people are normally optimistic, they can think about how to get past the barrier or the problems, even if they don't actually achieve it when it comes to the actions themselves. Optimism is good. But optimism, especially group-think optimism, can lead to some spectacular disasters.
Anyone who has attended one of my talks or training courses will know that courageous leadership is something I feel very passionate about.
What does safe mean to you? The dictionary defines safe as “protected from or not exposed to danger or risk; not likely to be harmed or lost” and in the context of many workplaces, we often think about the physical risks. These can include slips, trips, falls or much larger risks such as process safety failures leading to catastrophic accidents with multiple injuries or fatalities. These are all credible negative outcomes which we should be concerned about and so built into the risk management plans we have (which should be constantly reviewed and updated).
Human Performance is a well-established performance improvement method which has been used in high hazard industries such as Aviation, Nuclear Power Generation and Space Exploration for many years to try to manage the issue of 'Human Error'.
